Privacy at Historia
Historia handles the information needed to document Encounters and operate outpatient care.
Responsible party
The Historia team is responsible for operating the Historia service and maintaining this privacy notice. For questions about privacy or this notice, email privacy@historiamd.com.
Information the product processes
This may include patient demographics, Appointments, Transcripts, Canvas content, Key Points, Drafts, finalized clinical records, and files the team attaches.
Purposes and basis of processing
We process data to provide the electronic health record service and support health teams’ operations. Historia performs this processing on behalf of the clinical Workspace, under its instructions and the legal bases it determines under applicable law.
Access scope
Patients belong to the Workspace. Its Clinicians share access to Patients and clinical records; reception staff are limited to the Schedule and demographics needed to assist the Patient.
Retention
Data is retained while the Workspace maintains its Historia account. Deletion requests are handled through the Workspace, subject to any applicable legal retention obligations.
Individual rights
Individuals may request access to, correction of, or deletion of their data where applicable, in accordance with the laws of the Workspace's country. Requests may be submitted to the treating clinic or to privacy@historiamd.com so we can route them to the responsible Workspace.
Country-specific provisions
Each Workspace is created with a country, which determines the applicable legal framework. In Chile, individual data rights are governed by Law 19.628 and the clinical record by Law 20.584; these provisions apply only to Chilean Workspaces.
Processors and transfers
We use infrastructure providers, including Cloudflare for hosting and storage, and AI transcription providers when clinical audio is processed. Some processing may occur outside the Workspace's country, with contractual and technical safeguards appropriate to the sensitivity of the information.
Use of AI
Historia uses AI to organize documentation. It does not diagnose, recommend treatment, or replace clinical judgment. Key Points are strictly extractive.
